Photoshop Paper

The first step is to create a lined paper pattern.

Create a new document roughly 20 pixels high and 1 pixel wide. Using a 3 px brush draw a dot in the middle of the document, next go to edit > define pattern..., name the pattern whatever you want. Close the document.

Create a new document, reasonably sized to fit your notepad. Make a new layer and use the marquee tool to draw a rectangle and then fill it with grey. Then right click on the layer in the layers panel and from the context menu choose Blending Options... Then go to pattern overlay and choose the pattern that you originally created. Lower the opacity settings slightly to give the paper a more realistic appearence. Here are the settings I used.

You now have a simple piece of paper lacking in depth. To create the appearence of depth duplicate the current layer. Now in the layers panel, take lower of the two layers and right click on it and from the context menu choose Blending Options... then use the following settings for gradient overlay and dropshadow

Now go to Edit-> Free Transform (Ctrl + t) and slightly increase the size of the bottom layer. Position the layer so that there is a little bit of the black part sticking out of the top, and a lage chunk of off-white at the bottom. When you are done it should look something like the image to your right.
